Sonographic Characteristics of Injectable Agarose Gel Filler: Two Case Reports and Practical Insights From Ultrasound-Guided Aesthetic Practice

Abstract
Background:
High-frequency ultrasonography has become an essential adjunct in modern aesthetic practice, particularly for improving the safety and accuracy of injectable fillers. While the sonographic appearance of commonly used fillers is well described, limited data exist regarding agarose-based gel fillers.
Objective: To describe the immediate in-vivo ultrasound appearance of injectable agarose gel filler in the temple region and to correlate imaging findings with procedural technique and tissue integration.
Methods: Two patients undergoing agarose gel filler injection for temporal hollowing were evaluated using real-time, high-frequency ultrasound before and immediately after injection.
Results: Agarose gel filler demonstrated a well-defined, predominantly isoechoic appearance with minimal acoustic artifact. Integration with surrounding tissues varied depending on timing and injection dynamics.
Conclusion: Agarose-based fillers exhibit a distinct and reproducible sonographic pattern. Ultrasound guidance provides valuable real-time feedback during injection and enhances understanding of filler behavior in vivo.
